What care do you want at the end of your life?

The best time to describe your wishes for the end of life journey is now. What do you want? What don’t you want? Begin by having important conversations with the people who mean the most to you. This will help your loved ones make more confident choices for you, should they be in the position of decision making on your behalf.

Share your beliefs and values about end of life care with your family, friends, caretakers, and physicians. Next, document these desires in advance directives. You should include a living will and a POST found here.
